Discrimination Incident Report Form

Communicating a transparent reporting and complaint resolution process for incidents of racism or discrimination.

To provide an environment that is safe and free from racism or any form of discrimination a club must have a clear reporting system. This must be supported by leadership that communicates the importance of reporting and a clear process to ensure they are heard and addressed.

Direct intervention or confrontation in a situation where discrimination is occurring is daunting for most people. Fear of making things worse and fear for ones own safety or the safety of others is a significant barrier to direct engagement with a perpetrator.

An incident report form, specifically for incidents of discrimination ensures incidents are addressed, perpetrators are challenged on their behaviour and are provided with an opportunity to learn and be better. It also shows victims that they are supported by the club community. Many victims do not report incidents themselves because they don’t think anything will be done.

The Welcoming Clubs Incident Report Form is a generic form that your club can copy and paste, sharing on your social media pages or website so that everyone in your club community knows what steps to take if they witness or experience discrimination.
